HCP-16 stacked on the HCP-15 session-history slice. Instruments session open/close at the shared runtime boundary, propagates generation session identity, records explicit close reasons for client close, host shutdown and memory pressure, and keeps telemetry failures best-effort. Temporary implementation runner files will be removed before review.
