Adds the combined review and eval caller and the manual re-run caller, both pinned to infra-ai-platform v2.12.1, plus a CODEBASE.md draft when the repo had none; removes the legacy workflow_run eval pair when present. Opened by the onboarding fanout of infra-ai-platform.

Adds the AI review callers pinned to infra-ai-platform v2.12.1: the combined review and eval caller and the manual re-run caller, plus a CODEBASE.md draft where the repo had none (an existing CODEBASE.md is kept as is). The review is informational: a non-Approve verdict turns its check red, merging stays possible. An org admin can switch it off for this repo at any time by setting the custom property ai-review to off, or org-wide via the AI_REVIEW_DISABLED variable.
