Outcome: a reviewer can understand, evidence, resolve and record a scoped final human disposition without erasing history or allowing an automated clearance path.

Scope: TS-401 through TS-405; existing issue #8.

Requirements: FR-CAS-001 through 008 and FR-AUD-001 through 002.

Exit evidence:

• P0/P1/P2 state-machine and forbidden-transition tests
• Append-only evidence/finding events
• Authorized clear/block/no-action decision, scope, expiry and invalidation
• Minimal accessible reviewer flow
• Redacted decision-first audit/evidence pack

Risks: reviewer authority/four-eyes unresolved, inadequate human intervention, audit mutation, usability/workload.
