chore(ci): remove remaining dead-app refs (badges, shell uploads, docs)

Follow-up to the merged Codecov sweep PR: coverage uploads performed via shell blocks (curl uploader.codecov.io, ./codecov), README badges and doc claims are not YAML steps, so the automated step rule could not reach them. No CI behaviour change beyond dropping references to a service that is not installed.

Second pass added: coverage uploads done via a shell block (curl uploader.codecov.io / ./codecov) and badge/doc references, which a YAML-step rule cannot see. Verified by re-grepping the pushed branch for uses: codecov, CODECOV_TOKEN, uploader.codecov.io, codecov.io/gh/ and img.shields.io/codecov.
