docs(plans): ultraplan and cross-repo hand-offs (plan only)

Summary

Adds the recon-based plan for bringing this repo in line with the standards repo and the RSR template format, plus the path for issues #105, #104 and #84, and the hand-offs to other repos and bots.

This PR is docs only. It changes nothing outside `docs/plans/`. No A2ML is converted by hand, and every migration step is gated on the owner decisions listed in the ultraplan.

• `docs/plans/ULTRAPLAN-2026-10-09.adoc`: evidence baseline, standards/RSR compliance gaps, phased paths for [Feature]: combinatorial tactic playbook synthesis using delta/partial neighborhood evaluation (ParadisEO-mo) #105, [Feature]: anti-pattern mining and objective fitness formulation from burrow.jsonl for heuristic guidance #104 and Track unfinished embedded AffineScript compiler, runtime, tooling and provenance #84, sequencing (W0–W5), acceptance, risks, owner decisions D1–D10.
• `docs/plans/CROSS-REPO-HANDOFFS-2026-10-09.adoc`: draft briefs for standards, rsr-template-repo, echidna, echidnabot, hypatia, the fleet bots and affinescript. Nothing is filed in other repos yet.

Key findings

• The repo is far from the template format: 119 tracked `.a2ml` files, placeholder tokens in 13 files, and `just validate-rsr`, `validate-state` and `verify-template` all failing.
• [Feature]: combinatorial tactic playbook synthesis using delta/partial neighborhood evaluation (ParadisEO-mo) #105 and [Feature]: anti-pattern mining and objective fitness formulation from burrow.jsonl for heuristic guidance #104 are blocked on echidna's stepwise proof-state surface. `apply_tactic` returns a clone of its input, and the proof state is raw file text, so sub-goal counts and step caching are not available yet.
• Track unfinished embedded AffineScript compiler, runtime, tooling and provenance #84 is misfiled here: `affinescript/` has no commits in this repo's history.
• The RSR baseline is a file-presence proxy, not the normative oracle (`hypatia:rsr-conformance`), which is not implemented yet.

Open items (not resolved by this PR)

• `proof-safety.yml` run 37371572643 on `main` (2026-10-05) failed with a cancelled job and no retrievable logs. Cause unknown.
• Owner decisions D1–D10 (ultraplan §7) are required before any migration.

Test plan

• `docs/plans/` is the only path changed (`git diff --stat origin/main...HEAD`)
• Reviewer reads the ultraplan §5 acceptance checklist
• Owner resolves D1–D10 before W1+ work starts
