chore(ci): drop the Codecov upload step from the repo template and wiki examples

The Codecov GitHub App was uninstalled from this estate (owner ruling, 2026-09-20 audit);
these were the last references to it in this repository. Coverage generation is unchanged —
only the upload to a service this org no longer uses is removed, so scaffolded repos stop
inheriting a step that can never authorise.

No .github/workflows/*.yml file changes here, so no actions.lock regeneration is due:
nothing in this diff can add or remove a lock-keyed uses: ref.

Found by the estate ghost-app audit; part of the same sweep as the 40 merged PRs and standards#885.
