Changes to use an address resolver which resolves the next address to attempt for a connection. This is towards issue #17 (https://github.com/jhalterman/lyra/issues/17)

Move AddressResolver and DefaultAddressResolver to config package. Changed the AddressResolver to the second option that was discussed since it is more adaptable to underlying AMQP library.
