Make ADHD style automatic in Claude and Codex

• add a forced Claude Code output style so enabled plugins shape every main response
• front-load the portable skill trigger and document the global Codex AGENTS.md default
• add one-answer opt-out with ADHD off for this answer:
• replace the README attribution with A dME Inc. and an MIT license
• show code and research/report examples directly on the GitHub README
