refactor(web): migrate console icons to Hugeicons

Summary

• replace every generic Mosoo Console UI glyph from Lucide with a local Hugeicons Free adapter
• preserve branded, provider, channel, runtime, and illustrative SVG assets
• use the shared cross-product rendering contract: currentColor and 1.5px stroke
• correct the MCP credential-revocation affordance to a separated plug/socket glyph
