This commit modifies the ToolGroup API to allow retrieving MCP server instructions, that are usually advertised in MCP initialization response. https://modelcontextprotocol.io/specification/2025-11-25/basic/lifecycle#initialization Also adds this to StreamingResponseOrchestrator.
