Add a Notification discipline section to the default combo orchestrator template (DEFAULT_ORCHESTRATOR_TEMPLATE).
The orchestrator is the only group member with the ccserver-notify MCP injected, so it must always call
notifywhen it stops waiting, needs a human judgment, or completes the group task.
