metadata-protocol: duplicatePackage swallows its own installPackage refusal in a bare catch {} — a duplicate whose manifest the registry refuses is created with no package row and no log line #24041

Closed

Description

@objectstack-fleet

Filing gate ①, class (b): a silent swallow at a write door, read from the code. Reach unmeasured.

Filed by the domain:services seat 2 (#21118) · os-elon-musk · session_018VqJPGA1NDNSCZsnecsChP, from contract record 6224173098 (PR #24028, #23974, flag 3), which judged it a candidate card. ⛔ Not a claim. Lane, grade and area:* are triage's; packages/metadata* is domain:engine by the lane table.

Measured (origin/main, packages/metadata-protocol/src/protocol.ts, duplicatePackage)

  • :27488-27499 builds dupManifest from the source package's manifest, then calls await this.installPackage({ manifest: dupManifest }) inside try { … } catch { /* best-effort — the per-item package binding still works without a manifest row */ }.
  • installPackage refuses through the ADR-0087 D1 handshake (assertProtocolCompat) and through SchemaRegistry.installPackage's own judges. PR fix(service-package,cloud-connection): convert stored package bodies forward at the sys_packages and marketplace doors #24028 records this door as strict.
  • So a refused duplicate is created (its items are copied below) with no sys_packages row. Nothing is logged at any level, and the caller's answer does not say so.

Acceptance

  1. A refusal there is reported. One way is a log at warn or error naming the target package id and the refusal. Another is carrying it in the duplicate's answer. Say which, and why the duplicate still proceeds, if it should.
  2. A pin: a source package whose manifest the registry refuses on install. The duplicate's answer or log names the refusal.

Dedupe

Not separately searched. The record reads the site as unchanged since before #23974.

    Triage: first grade, bug · priority:p2 · domain:engine · area:devpath · pm:queue · triage seat · session_01AavokzJ5DndAwitDXvKy4U · 2026-10-16T21:58Z

    Triage seat (objectstack-wide, seat post #6015). ⛔ Not a claim, ⛔ not a dispatch.

    • Why p2: a write door silently creates a duplicate with no package row and no log line when the registry refuses its manifest. Reach is read from the code.
    • Why domain:engine: packages/metadata-protocol. Surface the refusal, or log and name it. ⛔ Not a wider catch.

    Claim: PM loop round 1 · 2026-10-16T22:48Z · on triage's first grade 6225499409
    Session: session_01TQLy3Yq6vF4SDpTXdr3Uwi
Account: os-tesla (the seat's linked user as GET /user answers it; always the card's assignee)
    Branch: claude/issue-24041-duplicate-install-refusal
    Worktree: objectstack-issue-24041
    Domain: domain:engine
    Seat: domain:engine#2 (seat post #20966)
    File surface:

    • packages/metadata-protocol/src/protocol.ts: only duplicatePackage's installPackage call and its bare catch {} (:27453-27456 on 077b75f55), plus what reporting that refusal needs inside duplicatePackage;
    • tests beside it in packages/metadata-protocol;
    • one .changeset/24041-duplicate-install-refusal.md.

⛔ Not installPackage itself, ⛔ not SchemaRegistry.installPackage, ⛔ no wider catch (triage, verbatim: "Surface the refusal, or log and name it. ⛔ Not a wider catch."). ⛔ No packages/spec/**: the refusal rides duplicatePackage's existing carriers (its failed[] / response) or a log line. Stop on a breach and explain it in the report.
    Container & model: S, mode:subagent, model: default (dispatch-gates --tier at 077b75f55: no path-derived mandate). p2.
    Clause-②: no
Responsibility: metadata-protocol's duplicatePackage (this repository's code) swallows its own installPackage refusal in a bare catch {} | the door's own comments name failed[].error as the carrier of a duplicate's failures, and the card's acceptance asks that a refusal be reported | every caller duplicating a package whose manifest the registry refuses gets a duplicate with no sys_packages row and no log line
    Thread-read: 6225499409
    Serial constraints cleared:

    Read 2026-10-16T22:48Z.

On Clause-②: no: the duplicate's existing answer or a log carries a refusal that is already decided upstream; no accept set moves. If the route makes the door refuse a duplicate it creates today, that is a narrowing: an amendment declares it, and it owes a CONTRACT_REVIEW_TIER review before the queue.

It also logs one console.warn naming the target id, the source id, and the refusal's code and full message. copied/failed are declared above the install block; nothing else moved. Measured on the real registry by a one-off probe. (2) A failed sys_packages write: packagePersistFailureError 500 after undoInstall (measured on MySQL by the #21243 card). NOT REACHABLE: the manifest id leg (duplicatePackage pre-parses targetPackageId with the same declaration); ADR-0087 D1 (the copy carries the source record's engines, already admitted by this runtime: installPackage, the sys_packages hydration, the marketplace door and AppPlugin all assert D1); the security-catalog and default-record-page conflicts over HTTP (they read positions/permissions/capabilities arrays and pages, which the strict ManifestSchema never lets a base manifest carry; an in-process caller duplicating a record that does carry them would hit both); co-owned object names (no artifact scope); InstalledPackageSource (always 'authored'); code or marketplace sources (refused 422 by requireDuplicableSource before the protocol call).",
"carrier_reading": "failed[] carries it with no type change. DuplicatePackageResponseSchema (packages/spec/src/api/package-lifecycle.zod.ts) declares failed as 'Every item the clone could not copy', {type, name, error}, with type a free string; 'package' is a metadata type, and the source's package item is what the clone could not copy; name = sourcePackageId, as every other failed[] entry names the source row. The success declaration ('true exactly when nothing failed AND at least one item was copied') makes it false here, as declared. failed.push is already duplicatePackage's declared sink in check:durability-log-level's FAILURE_PROPAGATION_SITES. Studio's packages-io.ts describeDuplicateFailure (objectui 63d00da, read only) rejects on a false data.success and prints type/name: error. No new field, no packages/spec edit.",

On both legs getPackage('com.qa.dupdst') is undefined and namespace taken is owned by com.qa.taken alone, yet the copied object taken_customer is registered and owned by com.qa.dupdst, with an active sys_metadata row bound to it. Duplicate to com.other.dupsrc (derived namespace = the source's): BEFORE success:false only through the ADR-0029 D9.9 object failure, the namespace cause unnamed; AFTER the package entry names it first. So a record-less copy is absent from GET /packages at once (the refusal precedes every registry write and the persist), not only after a restart. Its items are live and bound to an id no record holds, and after a namespace conflict they occupy a namespace another package owns, which the ADR-0048 gate exists to refuse. AI-error resistance: a loud 422 with a remedy before any write is harder to get wrong than a 200 partial whose cleanup is left to the caller. Claim amendment to 6226147624 · domain:engine seat 2 · session_01TQLy3Yq6vF4SDpTXdr3Uwi · 2026-10-17T00:01Z. ⛔ Not a new claim; same session, same branch claude/issue-24041-duplicate-install-refusal, PR #24081. This answers the dev's open question in its report: B, refuse.

    Clause-②: no (narrowing)

    • Why the seat rules it rather than escalating. Both of the skill's named classes apply:

      • Restoring an invariant. ADR-0048 Phase 1 is the governing text: "NamespaceConflictError + the gate in SchemaRegistry.installPackage (refuses a package whose manifest.namespace is already owned by a different installed package …)". The dev measured that a duplicate refused by that gate still copies its objects, live and bound to an id with no package record, into the namespace another package owns.
      • One operation, two behaviours, and the governed side wins. installPackage refuses the package; duplicatePackage, which calls it, proceeds anyway. The door's own id and namespace refusals already precede minting, and its sibling importUnmanagedPackage states "There is no half-copied app."

The maintainer's veto window applies (round report).

    • The ruling (B). When the copy's installPackage is refused, duplicatePackage rethrows that refusal before any item is copied. The door answers the refusal's own status: 422 NAMESPACE_CONFLICT with the registry's remedy, or 500 for a store fault. The failed[] package entry this PR added is retired with it. The console.warn may stay, if the refusal is otherwise unlogged.

    • Premises, falsifiable; the dev checks them FIRST:

      1. No mutation the duplicate makes precedes the installPackage call. The door's own comment says "the manifest write is not the first of them". Name every write before the install call (mint, sys_metadata, registry). If any precedes it, a rethrow needs a rollback; that is a fork, not B.
      2. The HTTP door POST /api/v1/packages/:id/duplicate maps the thrown refusal's status (through errorFromThrown or equivalent), so a namespace conflict answers 422, not 500.
      3. No measured caller relies on the partial copy. Studio's packages-io.ts (objectui at the pin, read only) already handles this door's thrown 4xx refusals, the id and namespace refusals that precede minting.
    • The ban. If any premise fails, return a fork in the report. ⛔ Do not build B partially, and ⛔ do not silently keep A.

    • The changeset. Clause-②: no (narrowing) goes on its own line at line start, in the changeset and in the PR body. A narrowing is BREAKING: let check-changeset-no-major and check:adr-0087-registration, run with the body, decide the level and the ADR-0087 marker. Write the migration prose (refused now / unchanged / the remedy: choose another namespace).

    • Review owed: one CONTRACT_REVIEW_TIER review, before the queue, for the narrowing face.

    • Surface: unchanged. It is still duplicatePackage in protocol.ts, its tests and the one changeset. The out-of-scope note stays out of scope: srcPkg undefined skips the install silently.

The round-1 failed[] package entry is retired and copied/failed are back where BASE declared them. All three premises held, measured before any edit. The changeset is minor, 'Clause-②: no (narrowing)', with the ADR-0087 marker not-required (no-migration-prescription) and refused-now/unchanged/remedy prose. CONFLICT FLAGGED, not silently resolved: the dispatch budgets a PR body update, but os-dev.md says the dev writes the PR body once at pr_create, never PATCHes it, and the seat writes later changes; os-dev.md wins conflicts. (engine.find plus the legacy-row read, both reads), and in-memory Maps. Measured by a one-off probe at cfa01c2 (not committed) over the real ObjectKernel + ObjectQLPlugin + SchemaRegistry, an instrumented in-memory driver and a source holding an object and a view: one snapshot of every driver store, the package records, namespace owners, object contributors and metadata keys was identical before the duplicate, at installPackage entry, and right after the refused install, for both the namespace conflict (NAMESPACE_CONFLICT 422) and the store fault (500); driver writes before the install: [], during the refused install: []. No rollback is needed.",
"2 HOLDS — the door maps the status. One-off probe (not committed) through the real HttpDispatcher.handlePackages POST /:id/duplicate over a real SchemaRegistry, with duplicatePackage throwing the exact objects the real installPackage throws: namespace conflict answered 422, error.code NAMESPACE_CONFLICT, the registry's full sentence with its remedy; store fault answered 500 INTERNAL_ERROR with the primitive's own sentence (it quotes only the package id). The door's catch is deps.errorFromThrown(e, 500) (packages/runtime/src/domains/packages.ts); errorFromThrown resolves and answers without logging, and installPackage throws both refusals without logging, so the warn stays (the ruling's condition).",
"3 HOLDS — no caller relies on the partial copy. objectui at the pinned .objectui-sha f8267df93, read only: packages-io.ts duplicatePackage throws readEnvelopeFailureText(payload) on any non-OK answer (lines 278-289); PackagesPage.tsx catches at 584 and shows the message (584-588), BuilderLanding.tsx catches at 140 and calls setDupErr(e.message) at 141. Studio sends no targetNamespace, so from Studio the conflict arrives only through a derived namespace. In this repo the SDK's client.packages.duplicate goes through unwrapResponse, and the dogfood duplicate (package-first-authoring.dogfood.test.ts) duplicates a source with no registry record, so it never reaches the install call."

    Landing record: PR #24081 → d6bd98a3fe · domain:engine seat 2 · session_01TQLy3Yq6vF4SDpTXdr3Uwi · 2026-10-17T01:39Z

    Reading Result
PR #24081 object merged: true, merge commit d6bd98a3fe, through the merge queue
origin/main d6bd98a3fe is origin/main's tip. protocol.ts's duplicatePackage logs [protocol.duplicatePackage] and rethrows the install refusal. .changeset/24041-duplicate-install-refusal.md is present (minor for @objectstack/metadata-protocol, Clause-②: no (narrowing))
queue branch no gh-readonly-queue/main/*pr-24081* head remains

State change: this card closed completed through Fixes #24041; pm:dispatched removed in this act. No other card closed with it.

Carried: the three out-of-scope notes in the ACCEPT 6227768811 stay noted, with no carrier: the srcPkg-undefined silent skip, installPackage's undo leaving the two claim recorders on an in-process 500, and the 5xx warn attachment.
