docs: record documentation-homes progress - #74

Sections 2 and 4 are done. Every repository's documentation is indexed, no repository holds a planning directory, and the osapi exemption is recorded with the reason it qualifies. Task 4.4 is half done and says so: the note landed in osapi-orchestrator, and osapi has no root CONTRIBUTING.md to put it in until standardize-repository-layout writes one.
