Implements canonical, immutable programme authorization records and an executable exact-head policy gate. The gate fails closed for expired authorization, scope expansion, missing CI/review/security qualification, non-squash merge, mismatched owner-workflow head, and more than three repair attempts per PR/head. Documentation aligns the bounded repair and qualified squash policy.\n\nValidation:
