Twelfth batch of the adversarial review loop (Codex, an OpenAI model; affiliation noted in every dated note, rule-4 note on 0025). Primaries re-read for this PR: OpenAI's expanded sycophancy postmortem, the arXiv listing, crypto.news on aixbt.

  • 0023 aixbt: prompt-injection removed as a contributing cause. The maintainer's statement and independent coverage describe unauthorised dashboard access and two queued malicious replies; nothing shows the attacker manipulating model input rather than controlling the dashboard. The record no longer implies a jailbreak.
  • 0021 Grok/Bankr March 2025: direct_loss_usd to unknown; the ~$330K figure appears only in May 2026 retrospectives with no transaction reconstruction and stays as a reported upper anchor; root_cause marked as the reported mechanism; the March launch/block event kept distinct from the May 2026 NFT escalation.
  • 0025 GPT-4o sycophancy: the expert-tester flag and the decision to launch on positive A/B signals are now quoted from the postmortem ("should we withhold deploying this update ... based only on the subjective flags of the expert testers? In the end, we decided to launch"); rollout dated 04-24 to 04-25 per OpenAI; "100% of live traffic" and the ~3-day exposure phrased as OpenAI's account.
  • 0024 Cursor "Sam": "triggering churn" removed (anecdotal); the ~$100M+ ARR figure removed (unsourced, unneeded); "no human review and no AI label" reworded to what the company admission supports.
  • 0022 ElizaOS: date_disclosed 2025-03-20 (arXiv v1); the Sepolia amount, mainnet repeat, cross-platform propagation and shared-memory claims marked as full-text and press detail rather than abstract; researcher-owned funds distinguished from third-party loss.

INDEX: three rows updated; enum counts unchanged.

0023: prompt-injection removed as a contributing cause; the maintainer's contemporaneous statement and independent coverage describe a dashboard-access hack, not manipulation of the AI. 0021: direct_loss to unknown with the ~$330K retrospective figure as a reported upper anchor; root cause marked as the reported mechanism; March 2025 launch/block kept distinct from the May 2026 escalation. 0025: expert-tester flag and the launch-on-A/B decision now quoted from OpenAI's expanded postmortem; rollout dated 04-24 to 04-25 per OpenAI; '100% of traffic' and the 3-day window phrased as OpenAI's account. 0024: 'triggering churn' and the unsourced ARR figure removed; 'no human review / no AI label' reworded to what the admission supports. 0022: arXiv v1 date 2025-03-20; full-text specifics distinguished from the abstract; researcher funds distinguished from third-party loss. Reviewer affiliation (Codex, an OpenAI model) noted; rule-4 note on 0025. INDEX rows updated.
