refactor: update config for neovim 0.12

• Replaced packer.nvim with a custom vim.pack-based plugin loader.
• Replaced nvim-cmp setup with native completion, including built-in LSP completion and a custom filesystem path completer.
• Fixed treesitter config
• Misc cleanup of unused plugins and methods
