fix(docs): reconcile stale docval backlog with current evidence (PLF-010)

The historical docval export still marked resolved findings as open, including four critical broken links to content no longer present. A fresh scan supersedes 152 historical records (one was already marked done in the primary checkout) and preserves 31 still-reported records with source hashes and candidate locations. Repeated headings remain distinct tickets. Correct the stale analysis.toon artifact references to analysis.toon.yaml.

Add a conservative revalidation script that runs a fresh scan, rejects documentation changes during scanning and preserves unscanned/ambiguous records. Severity stays separate from evidence of present urgency. The remaining candidates stay available for the broader documentation review in #10.

Validation: full suite 56 passed; focused reconciliation tests 2 passed after freshness guard. The existing optional badge hook reported an unrelated installed tokenizer error; commit completed and README was unchanged.
