Add Ollama provider support

Summary:

• Add Ollama provider descriptor, settings, fetcher, snapshot, and parser
• Register Ollama across settings, menus, widgets, and CLI selection
• Add Ollama icon and docs, plus usage parser and fetcher tests
• Fix Ollama usage regex handling and add a capitalization regression test
• Ignore .swiftpm-cache in git
