Issue description

The NestedForm inside a Repeater on the Records page in the Test plugin has an issue where when creating a new repeater item, the name is inherited from the parent form because they both contain a name property.

Steps to replicate

1. Install test plugin
2. Go to records page
3. Add the required image and name then save the page
4. Reload the page
5. Add a new repeater item for the "Standard Repeater"
