Support cover images and transient AI retries

Summary

• Move leading Markdown cover images into the cover page and prevent duplicate image-caption rendering.
• Allow cover pages to render optional images and image-caption pages to fall back to a text card when the image is moved.
• Retry transient AI command failures such as startup lock contention before surfacing an error.
