docs: split Codex Claude integration evidence

Summary

• Add docs/CODEX_CLAUDE_LOCAL_INTEGRATION_EVIDENCE_2026-07-06.md for non-operator Codex/Claude integration evidence.
• Expose codex_claude_local_integration_evidence in the 9.5 evidence script and tests.
• Keep native_dialog_approved_dogfood pending until explicit operator approval; this PR does not run the native approved dialog path.
• Package and guard the new evidence doc so release artifacts include the same local proof ledger.

Effect Check

• promptlane_95_quality remains pending.
• Blockers remain: Codex/Claude axis below target, web UI/operations below target, scheduled UI patrol, native dialog approved dogfood.
• Codex/Claude axis now distinguishes completed local evidence from the external operator-approved dogfood blocker.
• This prevents over-claiming 9.5 while still making verified local integration progress visible.
