chore: move to the hawserhq org - #18

PREPARED, NOT LIVE. Merge once zcsizmadia/hawser-vscode has been transferred to hawserhq. URLs in package.json, the README and the "install Hawser" link the extension opens when it cannot find the binary. One field here is not a URL: `publisher` is the VS Code Marketplace identity, and it has to be a publisher that actually exists and that we control. This commit assumes "hawserhq" will be registered. Nothing has been published yet, so there is no rename problem -- but the name has to be claimed before the first `vsce publish`, and the Marketplace namespace is separate from GitHub's, so owning the org does not reserve it.
