What changed
Introduces a single-owner instruction lifecycle, a non-normative incident catalog, executable instruction-contract validation, planning schema v2, atomic plan closure/archive operations, managed documentation indexes, and multi-axis command safety.
Why
Recurring pitfalls had become a second normative manual without a reliable chain from incident cause to canonical owner, loading route, executable guard, and retirement.
Impact
Target workflow upgrades now fail closed on customized or conflicting instruction owners, plan closure is mechanically validated, archive indexes are transactional, and safe diagnostic reads remain available while mutation and sensitive disclosure are blocked.
