Complete AgentTape hook coverage and CI validation

Summary

• cover all 11 current Codex hook events and serialize concurrent writers
• recover real Bash exit status from Codex transcript evidence while keeping transcript paths out of tapes
• add capture verification and directory-level regression execution to CI
• document a real Codex capture → inspect → fork → save → offline test validation

• real Codex CLI project: exit 7 captured as failed, redaction passed
