Rev B relay board: ask user for multiple 15-pin boards

For the Rev B relay board, we ask the user for the adapter board serial numbers of the "left board" and "right board."

However, for the Rev B relay board, we need to ask the user for the serial numbers of one 45-pin board and three 15-pin boards (for branches A, B, and C). Then, we need to add new columns to save this information in the output excel files.

The number and names of branches are based on the e-link type:

• Type 3.2: branches A, B, C
• Type 2.2: branches A, C
• Type 2.3: branches A, B
• Type 1.3: branch A

So, it would make sense for the user to only have to input serial numbers for the 15-pin boards that are used for that type of e-link. Then, when the data is saved, we should only save the 15-pin boards for that e-link type.
